This form is used to report incidents, near misses, hazards, or unsafe conditions identified during cleaning operations. Reports should be submitted as soon as reasonably practicable after the event or observation. Where immediate action has been taken to make the situation safe, this must be recorded. All incidents are reviewed to determine whether corrective action is required to prevent recurrence. Submissions are recorded against the reporting staff member as part of the CSS Cleaning Services LLP Integrated Management System.
